Form Embed Module Tutorial
Instructions
There are three types of form embeds available to choose from: Microsoft Forms, BlackBaud Forms, and other (custom embed script).
Creating a Form Embed

For this section you will need: 1 Form URL or embed script
Open the Microsoft Form you want to embed, select Collect responses, and copy the share URL provided in the top-right corner.
Return to Cascade and paste that URL into the Microsoft Form URL/Link field in the Form Embed module.

Cascade Instructions
On the Cal Poly Giving site, open the Giving Search page (You may access the page via the button below).
Use the search to find your department/program/college. In the results, select the correct listing and click Donate.
When the donation page loads, copy the full URL from the browser address bar.
Return to Cascade and paste that URL into the Blackbaud Form URL/Link field in the Form Embed module.
Save and submit/publish, then verify the form renders correctly on the page.
If your department or program does not appear in the Giving search list, please contact Michelle Magcalas at msmagcalas@cpp.edu to request that it be added. Once the department is available, you may use the provided Giving link to obtain the donation URL for embedding.
Other supported form platforms include Google Forms and Jotform. Additional form tools may also be used, provided they supply a valid HTML embed script that can be inserted into the form embed component.

Cascade Instructions
To obtain an embed script from Google Forms, open the form and select Embed HTML from the menu in the top-right corner. Copy the generated embed code, then paste the script into the appropriate Form Embed field in Cascade.
